I/We, the parents/guardians of the above-named candidate for a position on a GMLB team hereby give my/our approval to participate in any and all GMLB activities, including transportation to and from the activities
I/We know that participation in baseball may result in serious injuries and protective equipment does not prevent all injuries to players, and do hereby waive, release, absolve, indemnify, and agree to hold harmless the Greenfield Area Youth Minor League Baseball Incorporated, the organizers, sponsors, supervisors, participants, and persons transporting my/our child to and from activities from any claim arising out of any injury to my/our child whether the result of negligence or for any other cause
I/We agree that our child may be required to try out for a team, division or travel team. Such a try out does not guarantee that child a position on a team or in that division or on a travel team.
I/We understand that in addition to registration cost, players will have to participate in required raffle sales for the league/teams.
I/We , the parents/guardian of the above listed minor(s), hereby give permission to Greenfield Minor League Baseball (GMLB) to use photographs, names, voice recordings, or video taken of the above listed minor(s) during GMLB games and events, including team photos, for the promotion of league activities as determined in the sole discretion of GMLB. Such use could include but is not limited to, written or electronic publications, media releases, public announcements, website content, or social media posts. I/We agree that neither I/We, nor the above listed minor(s), will receive any compensation if such image appears in any of the manners listed above. I/We agree that such image is the property of GMLB.
